What are the EasyPay Copay Card Terms and Conditions?

What is the current EasyPay Copay Card offer?

The current EasyPay Copay Card offer varies by product:

  • CaroSpir: Eligible insured, insured patients without product coverage and cash paying patients will receive up to $75 off a 30-day prescription or $225 off a 90-day prescription of CaroSpir on their first and subsequent prescriptions.
  • Norliqva: Eligible insured, insured patients without product coverage and cash paying patients will receive up to $150 off a 30-day prescription or $450 off a 90-day prescription of Norliqva on their first and subsequent prescription.
  • Tadliq: Eligible insured patients with coverage will receive up to $1,000 off per 150 mL bottle of Tadliq on their first use and up to $750 off per 150 mL bottle of Tadliq on subsequent prescriptions. Eligible insured patients without product coverage will receive up to $1,900 off per 150 mL bottle of Tadliq on their first use and up to $750 off per 150 mL bottle of Tadliq on subsequent prescriptions. Cash paying patients do NOT qualify.
  • Atorvaliq: Eligible insured, insured patients without product coverage and cash paying patients will receive up to $150 off a 30-day prescription or $450 off a 90-day prescription of Atorvaliq on their first and subsequent prescription. Up to 12 bottles allowed per fill.
  • Liqrev: Eligible insured patients with coverage will receive up to $1,000 off per 122 mL bottle of Liqrev on their first use and up to $750 off per 122 mL bottle of Liqrev on subsequent prescriptions. Eligible insured patients without product coverage will receive up to $1,900 off per 122 mL bottle of Liqrev on their first use and up to $750 off per 122 mL bottle of Liqrev on subsequent prescriptions. Up to 6 bottles allowed per fill. Cash paying patients do NOT qualify.
Patient will be responsible for any remaining out of pocket expense.

Is it necessary to activate the EasyPay Copay Card before it is used at the Pharmacy?

No. The EasyPay copay card does not need to be activated prior to going to the pharmacy.

If a patient has insurance, but the prescription plan formulary does NOT cover the product, can the patient still participate in the offer?

Yes. If the insurance/prescription plan does not cover the product, the patient can receive the following:

  • CaroSpir: Up to $75 off a 30-day prescription or $225 off a 90-day prescription on their first and subsequent monthly prescriptions.
  • Norliqva: Up to $150 off a 30-day prescription or $225 off a 90-day prescription on their first and subsequent monthly prescriptions.
  • Tadliq: Up to $1,900 off per 150 mL bottle on their first prescription and up to $750 off per 150 mL bottle on subsequent monthly prescriptions.
  • Atorvaliq: Up to $150 off a 30-day prescription or $225 off a 90-day prescription on their first and subsequent monthly prescriptions.
  • Liqrev: Up to $1,900 off per 122 mL bottle on their first prescription and up to $750 off per 122 mL bottle on subsequent monthly prescriptions.
Patients are responsible for any remaining out of pocket expense.

What if the pharmacist recommends switching to another brand?

As long as you prescribe CaroSpir/Norliqva/Tadliq/Atorvaliq/Liqrev for your patient and include a note to “dispense as written” the pharmacy will dispense CaroSpir/Norliqva/Tadliq/Atorvaliq/Liqrev for your patients.

Can the EasyPay Copay Card be used with high deductible medical/prescription plans?

Yes. If a patient has a high deductible medical/prescription plan that includes coverage of CaroSpir/Norliqva/Tadliq/Atorvaliq/Liqrev, the patient should show the pharmacist both the valid prescription as well as the EasyPay Copay Card.

By having product coverage, a patient may receive:

  • CaroSpir: Up to $75 off a 30-day prescription or $225 off a 90-day prescription of CaroSpir on their first and subsequent prescriptions.
  • Norliqva: Up to $150 off a 30-day prescription or $450 off a 90-day prescription of Norliqva on their first and subsequent prescription.
  • Tadliq: Eligible insured patients with coverage will receive up to $1,000 off per 150 mL bottle of Tadliq on their first use and up to $750 off per 150 mL bottle of Tadliq on subsequent prescriptions.
  • Atorvaliq: Up to $150 off a 30-day prescription or $450 off a 90-day prescription of Atorvaliq on their first and subsequent prescription.
  • Liqrev: Up to $1,000 off per 122 mL bottle of Liqrev on their first use and up to $750 off per 122 mL bottle of Liqrev on subsequent prescriptions.

Can the EasyPay Copay Card be used if the patient hasn't yet met their prescription plan deductible?

Yes. A high deductible medical/prescription plan that includes coverage of CaroSpir/Norliqva/Tadliq/Atorvaliq/Liqrev can be used with the EasyPay Copay Card even if the patient hasn't yet met the prescription plan deductible. The patient will receive up to the maximum benefit for insured/covered patients. The insurance card must be shown to the pharmacist along with the prescription and the EasyPay Copay Card.

What if the pharmacist has problems processing a prescription for CaroSpir/Norliqva/Tadliq/Atorvaliq/Liqrev?

If the pharmacist has challenges processing a CaroSpir/Norliqva/Tadliq/Atorvaliq/Liqrev prescription, the pharmacist can contact the Pharmacy Help Desk at 1.800.433.4893.

Is the EasyPay Copay Card accepted at retail pharmacies?

Yes. The EasyPay Copay Card is accepted at participating retail pharmacies in the United States and Puerto Rico.

Is the EasyPay Copay Card accepted at specialty pharmacies?

Yes. The EasyPay Copay Card is accepted at participating specialty pharmacies in the United States and Puerto Rico.

Where can HCPs get the EasyPay Copay Card?

An HCP can get the EasyPay Copay Card at various medical conventions, in the mail, and online at www.EasyPaySavings.com/hcp or by calling 1.844.567.9503. Cards may be emailed, texted, or downloaded for print.

Where can a patient get the EasyPay Copay Card?

A patient can get the EasyPay Copay Card from their healthcare provider or by visiting https://easypaysavings.com/patient.

Can a patient have more than one EasyPay Copay Card?

Yes. A patient can have multiple cards, however, there is no need, only one EasyPay card is required.

Can the EasyPay Copay Card be used by patients with an existing prescription?

Yes, the EasyPay Copay Card can be used by patients with an existing prescription.

Can the EasyPay Copay Card be given as a gift to someone?

No. The card has no cash value, is not transferable, and can ONLY be used with a valid prescription for CaroSpir, Norliqva, Tadliq, Atorvaliq, or Liqrev.

If a patient does NOT have insurance, how much will the prescription cost each month?

Costs vary from pharmacy to pharmacy for cash paying patients, so the exact amount paid for their monthly prescription of CaroSpir/Norliqva/Atorvaliq while using the copay card can vary. The patient can receive:

  • CaroSpir: Up to $75 off a 30-day prescription or $225 off a 90-day prescription
  • Norliqva: Up to $150 off a 30-day prescription or $450 off a 90-day prescription
  • Atorvaliq: Up to $150 off a 30-day prescription or $450 off a 90-day prescription 
NOTE: Cash paying patients are not eligible for Tadliq or Liqrev savings.

What happens if a patient loses their card?

If a patient loses their card, they can receive a new card from you or can print a card out at www.EasyPaySavings.com

How much will a patient pay for a single, 30-day prescription of CaroSpir using the EasyPay Copay Card?

A patient will receive up to $75 off a 30-day prescription of CaroSpir. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.

How much will a patient pay for a single, 30-day prescription of Norliqva using the EasyPay Copay Card?

A patient will receive up to $150 off a 30-day prescription of Norliqva. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.

How much will a patient pay for a single, 30-day prescription of Tadliq using the EasyPay Copay Card?

An insured patient with product coverage will receive up to $1,000 off per 150 mL bottle on their first 30-day prescription of Tadliq. Insured patients without coverage will receive up to $1,900 off per 150 mL bottle on their first 30-day prescription of Tadliq. Subsequent 30-day prescriptions will receive up to $750 off per 150 mL bottle. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.

How much will a patient pay for a single, 30-day prescription of Atorvaliq using the EasyPay Copay Card?

A patient will receive up to $150 off a 30-day prescription of Atorvaliq. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.

How much will a patient pay for a single, 30-day prescription of Liqrev using the EasyPay Copay Card?

An insured patient with product coverage will receive up to $1,000 off per 122 mL bottle on their first 30-day prescription of Liqrev. Insured patients without coverage will receive up to $1,900 off per 122 mL bottle on their first 30-day prescription of Liqrev. Subsequent 30-day prescriptions will receive up to $750 off per 122 mL bottle. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.

Can this offer be used for patients eligible to be reimbursed under Medicaid (including Medicaid managed care), Medicare/Medicare Part D, TRICARE, Veterans Affairs, FEHBP, or any other government-sponsored prescription drug benefit plans?

No. Patients who are eligible for coverage of CaroSpir, Norliqva, or Tadliq product under any federal or state-sponsored programs are not eligible for this offer.

Is there an age restriction for the EasyPay Copay Card program?

No. The offer is valid for all patients when accompanied by a valid prescription.

When does the EasyPay Copay Card expire?

The EasyPay Copay Card program does not have a set expiration date. However, CMP Pharma reserves the right to amend, change or cancel the program at any time without notice.

How much does it cost to participate in the program?

The EasyPay Copay Card program is complimentary. There is no cost to participate in this program.

Is a prescription required for the EasyPay Copay Card program?

Yes. The EasyPay Copay Card is for use ONLY with a valid prescription for CaroSpir, Norliqva, Tadliq, Atorvaliq, Liqrev.

Can the EasyPay Copay Card be used for office visit co-pays?

No. The card can only be used to help pay for out-of-pocket expenses at the pharmacy with a valid prescription for CaroSpir, Norliqva, or Tadliq, Atorvaliq, Liqrev.

Can the EasyPay Copay Card be used for other purchases?

No. The card can be used to pay only for out-of-pocket expenses at the pharmacy with a valid prescription for CaroSpir, Norliqva, Tadliq, Atorvaliq, or Liqrev.

Is there a limit for how many times a patient can use the EasyPay Copay Card?

No. There is currently no limit to the number of times a patient may use the EasyPay Copay Card. The program may, however, be canceled or changed at any time without notice.

When the card is used, will personal medical data remain confidential, or will patient names and addresses be placed on a mailing list?

Can a patient use the EasyPay Copay Card with mail order pharmacies?

No. The EasyPay Copay Card cannot be used with mail order pharmacies. The EasyPay Copay Card must be used at point of sale at participating retail/specialty pharmacy locations.
