Is it necessary to activate the EasyPay Copay Card before it is used at the Pharmacy?
No. The EasyPay copay card does not need to be activated prior to going to the pharmacy.
If a patient has insurance, but the prescription plan formulary does NOT cover the product, can the patient still participate in the offer?
Yes. If the insurance/prescription plan does not cover CaroSpir, the patient can receive the up to $75 off a 30-day prescription or $225 off a 90-day prescription on their first and subsequent monthly prescriptions. For Norliqva, uninsured patients can receive up to $150 off a 30-day prescription or $225 off a 90-day prescription on their first and subsequent monthly prescriptions. For Tadliq, insured patients without product coverage will receive up to $1,000 off per 150ml bottle on their first prescription and up to $750 off per 150ml bottle on subsequent monthly prescriptions. Patients are responsible for any remaining out of pocket expense.
What if the pharmacist recommends switching to another brand?
As long as you prescribe CaroSpir/Norliqva/Tadliq for your patient and include a note to “dispense as written” the pharmacy will dispense CaroSpir/Norliqva/Tadliq for your patients.
Can the EasyPay Copay Card be used with high deductible medical/prescription plans?
Yes. If a patient has a high deductible medical/prescription plan that includes coverage of CaroSpir/Norliqva/Tadliq, the patient should show the pharmacist both the valid prescription as well as the EasyPay Copay Card.
By having product coverage, a patient may obtain the CaroSpir “Up to $75 off a 30-day prescription or $225 off a 90-day prescription” offer for their monthly prescription’. For Norliqva, the patient will receive the “Up to $150 off a 30-day prescription or $450 off a 90-day prescription”. For Tadliq, the patient will receive the ”Up to $1,000 off per bottle offer for their first month’s prescription” and receive the "Up to $750 off per bottle offer for their subsequent months’ prescriptions”. These monthly prescription costs will be applied to the patient’s deductible.
Can the EasyPay Copay Card be used if the patient hasn't yet met their prescription plan deductible?
Yes. A high deductible medical/prescription plan that includes coverage of CaroSpir/Norliqva/Tadliq can be used with the EasyPay Copay Card even if the patient hasn't yet met the prescription plan deductible. The insurance card must be shown to the pharmacist along with the prescription and the EasyPay Copay Card.
What if the pharmacist has problems processing a prescription for CaroSpir/Norliqva/Tadliq?
If the pharmacist has challenges processing a CaroSpir/Norliqva/Tadliq prescription, the pharmacist can contact the Pharmacy Help Desk at 1.800.433.4893.

Can the EasyPay Copay Card be given as a gift to someone?

How much will a patient pay for a single, 30-day prescription of CaroSpir using the EasyPay Copay Card?
A patient will receive up to $75 off a 30-day prescription of CaroSpir. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.
How much will a patient pay for a single, 30-day prescription of Norliqva using the EasyPay Copay Card?
A patient will receive up to $150 off a 30-day prescription of Norliqva. Any remaining out of pocket expense will depend on the patient’s insurance coverage and will be the patient's responsibility.
How much will a patient pay for a single, 30-day prescription of Tadliq using the EasyPay Copay Card?
An insured patient with product coverage will receive up to $1,000 off per 150ml bottle on their first 30-day prescription of Tadliq. Insured patients without coverage will receive up to $1,900 off per 150ml bottle on their first 30-day prescription of Tadliq. Subsequent 30-day prescriptions will receive up to $750 off per 150ml bottle.

Can this offer be used for patients eligible to be reimbursed under Medicaid (including Medicaid managed care), Medicare/Medicare Part D, TRICARE, Veterans Affairs, FEHBP, or any other government-sponsored prescription drug benefit plans?
No. Patients who are eligible for coverage of CaroSpir, Norliqva, or Tadliq product under any federal or state-sponsored programs are not eligible for this offer.
Is there an age restriction for the EasyPay Copay Card program?
No. The offer is valid for all patients when accompanied by a valid prescription.
When does the EasyPay Copay Card expire?
The EasyPay Copay Card program does not have a set expiration date. However, CMP Pharma reserves the right to amend, change or cancel the program at any time without notice.
How much does it cost to participate in the program?
The EasyPay Copay Card program is complimentary. There is no cost to participate in this program.
Is a prescription required for the EasyPay Copay Card program?

Can a patient use the EasyPay Copay Card with mail order pharmacies?
No. The EasyPay Copay Card cannot be used with mail order pharmacies. The EasyPay Copay Card must be used at point of sale at participating retail/specialty pharmacy locations.
